I hereby freely, of my own will and in my own interest, in accordance with Part 1 of Article 18 of Federal Law No. 38-FZ dated 13.03.2006 "On Advertising" and Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", give my consent to USHATAVA Trade House LLC, located at: 620142, Yekaterinburg, Shchorsa st., bld. 7, premise 22 (hereinafter — the Company), to send me advertising and informational messages (mailings) about the Company's products, services, promotions and special offers.

The consent is given to receive mailings via telecommunication networks, including:

• to the e-mail address provided by me during registration or checkout;
• to the mobile phone number (SMS messages);
• via messengers and push notifications.

For the purposes of sending mailings, I allow the Company to process, including with the involvement of third parties — mailing service providers, the following personal data: last name, first name, patronymic; e-mail address; phone number; information about orders and preferences.

The consent is valid from the moment it is given until it is withdrawn.

I have been informed that I may opt out of receiving mailings (withdraw this consent) at any time:

• by clicking the unsubscribe link contained in every e-mail;
• in the personal account on the website;
• by sending a request to info@ushatava.com marked "Opt-out of marketing mailings".

Opting out of mailings does not limit the ability to use the website and place orders.
